Where does the name come from? 

The system uses the Module Name + CRMID by default (e.g. Quote_4183.pdf  and the matching email of  Quote #4183) - you can change these details.

Here is an example of the name of a Quotes PDF as it appears by default:

and when sending the PDF via Email in OpenCRM you will see something like this:

What are my options for customisation?

You could either use a hard-coded name e.g. Your OpenCRM Quote or you can use merge variables to populate the PDF title with information held within OpenCRM for example the Subject of the sales entity -
